Who we are

The Suicide Loss Memorial Foundation is a 501(c)(3) nonprofit organization based in Greeley, Colorado (EIN 99-4094521). This policy covers information collected through suicidelossmemorialfoundation.org.

What we collect

We only collect what you give us directly. That includes:

  • Contact details you enter in a form on this site (name, email, phone, city).
  • Information you tell us in a message, for example, who you'd like to memorialize or what kind of help your family needs.
  • Donation records processed by our third-party payment processor, including the amount, date, and payment method.
  • Basic analytics about how pages are used (pages viewed, referrer, device type), collected through Google Analytics.

How we use it

  • To respond to your message or request.
  • To process a donation and send you a written acknowledgment.
  • To administer memorial placements and our youth program.
  • To keep records required by the IRS for a 501(c)(3) organization.

We do not sell your information or share your contact details with advertisers. We do use Google's analytics and advertising tools to measure our website traffic, as described under Cookies and analytics below.

Who we share it with

We share information only with the parties needed to do the work:

  • Our payment processor for donations.
  • A vendor or partner for a specific service you asked us to arrange, and only the information that service requires. For example, a parks department to place a memorial.
  • Our board of directors, when reviewing program and memorial requests.
  • Government agencies when required by law.

How long we keep it

We keep records for as long as they serve a legitimate purpose or are required by law. Donation and grant records are retained for at least 7 years for tax and audit purposes. General correspondence is retained for as long as the relationship is active.

Your rights

You can ask us to correct, access, or delete personal information we hold about you. Email contact@suicidelossmemorialfoundation.org with the request and we will respond within 30 days.

Children

Our forms are intended for adults. If a young person under 18 wants to reach us, we ask that a parent or guardian be involved in the message.

  • For our youth programs, we collect participant information directly from a parent or legal guardian with their consent.

Cookies and analytics

This site uses cookies required to operate, plus Google Analytics and Google Ads tools to measure how people find us and to run our advertising. You can turn off cookies in your browser settings. We do not sell your information to anyone.
