Path 1
Create a memorial for your loved one
For families who have lost someone to suicide and want a lasting place to remember them.
You choose the location, somewhere that meant something to your loved one, or somewhere you will actually want to visit. We handle the funding, the paperwork, and every conversation with the city, the parks department, or whoever else has to sign off. We serve families across Colorado.
Request a memorial
Tell us a little about the person you lost. There is no cost to your family, and there is no deadline. We will read what you send and get back to you.

Refer a young person to our Youth Social Engagement Program
For a young person aged 13 to 18 who has lost a loved one, or is struggling with their mental health, and would benefit from connection with peers.
An adult (a parent, grandparent, teacher, coach, counselor, or family friend) can make the referral using the form below. We will then contact the young person's parent or legal guardian. A parent or legal guardian must give consent before the young person takes part in any activity.
Please do not include the young person's phone number or email address on this form. We collect their contact details from the parent or guardian after consent.

Questions about requesting a memorial in Colorado
- Who can request a memorial?
- Any family in Colorado who has lost someone to suicide. Most of the families we work with are in Greeley, Weld County, and Northern Colorado, but we serve families statewide.
- How much does it cost?
- Nothing. We cover the bench, tree, or garden dedication and the permit costs. There is no fee and no expectation of a donation.
- Can I choose where the memorial goes?
- Yes. You pick the place, whether that is a Greeley park, a Weld County trail, or somewhere else in Colorado that meant something to your loved one. We handle the paperwork and the conversations with the city or parks department.
- How long does the process take?
- It depends on the site and the approvals involved. City and county reviews in Northern Colorado can take several weeks or longer. We keep you updated and do the chasing so you do not have to.
- How do I refer a young person to the youth program?
- Use the youth referral form on this page. It is for young people aged 13 to 18 in Greeley and Weld County, and a parent or legal guardian must give consent before they take part.
